【 信用卡授權書 / Credit Card Authorization Letter 】

本人因無法親自到達東京國際飯店刷卡結帳，特立此書，請在以下信用卡帳號收取保證金(一日房租)、預付或帳單款項。鑑於東京國際飯店同意上述之安排，本人同意不會取消此授權，並同意東京國際飯店可聯絡發卡銀行以下列信用卡所有人提供與核對此授權之資料。
I hereby agree to pay TOKYO INTERNATIONAL HOTEL with my credit card issuing this letter of authorization. Please charge the following credit card account for the deposit (one night rental), advance payments or room bills. I agree not to revoke the above authorization and further agree to authorize TOKYO INTERNATIONAL HOTEL to contact the credit card issuing bank as detailed below for the purpose or verifying the information contained in this letter.

*旅客如需更改訂房或取消訂房必需於入住前72小時通知 訂房部 更改或取消，否則東京國際飯店得收取一日房租。
*Cancellation will only be accepted in writing at least 72 hours advance notice. One night rental will be levied upon no show.
